How to avoid a hospital readmission?

Being hospitalized can be physically and mentally exhausting for both you and your caregiver.
Many times, people are readmitted due to complicacions caused by their health conditions.
These can be prevented and managed in the primary care physician's office.
It is important to recognize that the discharge process consists of severa! stages. It is necessary to prepare
for each stage in order to prevent a readmission.

Stage #I: In the hospital

- Make cure you understand che instructions about medications, nutrition, and the specific care you will
require at home.
- Ask your doctor if you should condnue taking any medications you have at home in addicion co those
prescribed at the time of discharge.
- Confirm that you and your caregiver understand the information received.
- Ask specific questions. don't leave any questions unanswered.

Stage #2: Transition to home

- Take your prescriptions co che pharmacy as soon as possible so that you have your medications available from day one.
- You should visir your primary care physician or specialist during the first seven days alter you are discharged from the hospital.

Stage #3: At home

- Adhere to your prescribed medication therapy in che hospital and follow your doctor's instructions upon
discharge.

hours a day. 7 days a week.
- Concinue co eat well and stay active until your physician has evaluaced you.
