As part of our recent achievements in the delivery of services and care, MCS Classicare obtained the highest score in the CAHPS survey for Health Plan and Drug Plan metrics in the United States and Puerto Rico, over 500 Medicare Advantage programs throughout the United States. For 2026, MCS Classicare became the only Medicare Advantage plan to achieve a 5 out of 5 star rating for three consecutive years, the highest score for quality of care awarded by the Centers for Medicare and Medicaid Services. It also became, for the second consecutive year, the plan with the largest membership in the United States to achieve this rating.

Regulatory Agencies

MCS maintains compliance with the agencies in charge of regulating the healthcare industry. Among these are:

  • The Office of the Commissioner of Insurance (OCI).
  • The Medicare and Medicaid Service Centers (CMS).
  • The Health Insurance Administration (ASES, by its Spanish acronym).
  • The National Association of Insurance Commissioners (NAIC).
  • Office of the Patient Advocate (OPP, by its Spanish acronym).
  • MCS also complies with the Final Rule for National Standards for electronic transactions (including Electronic Data Interchange Transaction and National Code Set Standards for claims processing) of the HIPAA Act (Health Insurance Portability and Accountability Act) of 1996.

Our Clients and Business Partners

MCS insures thousands of people in the service, distribution, manufacturing, and finance industries, among others.

The company has a broad network of providers comprising more than 15,000 doctors, dentists, laboratories, and hospitals, among others. In addition, it provides access to a wide network of more than 40,000 commercial pharmacies and over 50,000 Medicare pharmacies in Puerto Rico and the United States.

MCS maintains strong business relationships with various insurance brokerage firms in Puerto Rico and has independent agents active throughout the island.

Service Access Models

Open Network - Access to health services through a PPO (Preferred Provider Organization). This model is comprised of an extensive network of hospitals, physicians and other healthcare providers. You may freely access the services of physicians, general practitioners, specialists and subspecialists within this extensive network.

Coordinated Care - This model provides access to healthcare services through an extensive network of participating physicians in personal care specializations such as: Family Medicine, General Medicine, Internal Medicine, Obstetrics, Gynecology and Pediatrics. These physicians, in turn, coordinate the clinical care required with the various specialists and sub-specialists contracted by MCS.

MCS Through Time

1980's

1983

Medical Card System, Inc. (MCS) is born, thanks to the visionary efforts of the three founding partners: Felipe Benedit, José Durán-Ferrer and José Guillermo Meléndez.

1984

MCS obtains its first client: multinational insurer Transamerica Occidental Life Insurance. Others are rapidly added to MCS’s client portfolio.

1985

Nine (9) insurance companies are using MCS’s provider network, which by this date already had 3,419 providers and served 70,000 users.

1987

100% of MCS’s shares are acquired by Baxter Pharmaceutical. MCS opens offices in Florida to support Baxter’s operations in this state.

1990's

1993

MCS celebrates its 10th anniversary. The Baxter division, which was offering Caremark services, breaks away from the pharmaceutical. MCS separates from Baxter and becomes part of Caremark. MCS concentrates its operations in Puerto Rico.

1995

Already with 150 employees, and under the subsidiary MCS Life Insurance Company, MCS obtains its license from the Insurance Commissioner’s Office to provide life, health and disability insurance.

1997

A new acquisition puts an end to MCS’s relationship with Caremark when the latter merges with Med Partners. Six months later, Med Partners decides to sell MCS.

1998

The original founding partners decide to reacquire the business, together with other external shareholders.

1999

MCS reaches $40 million in earnings, insures 125,000 members, and ranks among the island’s top five most important insurance companies.

2000's

2000

The company begins to reorganize, in order to enter the Puerto Rico government’s Health Reform program, and creates the MCS-HMO subsidiary. MCS is already serving close to 1,000 private companies and is strongly positioned in the commercial segment.

2001

With 500 employees, MCS transcends traditional medical plans by incorporating its Wellness Program offerings, which integrate mental health, complementary and alternative medicine. It also begins offering the MCS Personal product.

2003

MCS celebrates its 20th anniversary.

2004

67% of the shares of Medical Card System, Inc., the parent company of the CS group of companies, are acquired by JLL Partners, a prestigious investment firm based in New York.

2005

As of January 1, the authorization granted by the CMS to market Medicare Advantage (Classicare) products becomes effective. MCS also receives approval from the Public Insurance Office to participate in the ELA market.

2011

MCS’s commercial business grows by more than 30%, with more than $240 million in annualized premiums. Jim O’Drobinak becomes the CEO of MCS. MCS also withdraws from the Puerto Rico Health Reform program.

2012

MCS launches its Group Life product. The MCS Advantage program receives a rating of three (3) out of 5 Stars.

2013

MCS celebrates its 30th anniversary.

2016

MCS creates a new subsidiary, MCS General Insurance Agency (GIA), to enter the life and disability product market. MCS Classicare reaches a rating of 4 out of 5 stars.

2018

MCS celebrates 35 years in Puerto Rico.

2019

MCS creates the MCS Classicare Te Paga card for its eligible members and integrates other innovative special supplemental benefits into its coverage to address the social determinants of health.

2021

The MCS Classicare Te Paga card continues to evolve, now with cash withdrawal and accepted in all merchants where they accept ATH. Additionally, MCS earns a rating of 4.5 out of 5 stars for 2022.

By December 2021, 100% of the shares of Medical Card System, Inc., are acquired by Kinderhook Industries, a private investment firm based in New York.

2023

MCS Classicare receives its first 5-star rating from the Centers for Medicare & Medicaid Services (CMS) for 2024. It also achieves the highest numerical score in the CAHPS survey for health plan rating and drug plan rating measures for all MA contracts within the United States and Puerto Rico. The MA plan announces that it received a score of 95% on both measures, once again demonstrating the quality of its health plan.

MCS celebrates 40 years of history and purposeful service. This is a momentous occasion to reflect on the transformative journey and unwavering commitment to offering quality services and genuine care to insureds, members, and the community of Puerto Rico.

2024

MCS Classicare receives a 5 out of 5 star rating from CMS for the second year, one of only nine plans to achieve this rating for 2025 among 521 Medicare Advantage plans evaluated in the United States. It also becomes the plan with the largest membership to achieve this milestone.

2025

MCS Classicare becomes the only Medicare Advantage plan to achieve a 5 out of 5 star rating for three consecutive years, the highest quality of care rating in the United States, awarded by CMS. It also becomes, for the second consecutive year, the plan with the largest membership in the United States to achieve this rating.
